CONFIDENTIALITY

Confidentiality Statement:
In order for your coach to help you fully, it is valuable, although not required, that you share your confidential and personal information, openly and honestly, in our comprehensive questionnaire and during your coaching sessions.

We will preserve the privacy and confidentiality of all of your personal information, including your name and email address, and all communications with your coach written or verbal. Only your coach will have access to your personal information, including your name, email address and written correspondence, including emails and questionnaire.

Your personal information will also not be shared with any person or organization including your employer, health plan, or healthcare provider, unless you provide permission.

We will maintain your client file for 12 months after the completion of your coaching program. We will delete and/or shred your client file at any time if requested by you.
